Applying for disability services can feel overwhelming — but you don't have to do it alone. Our team will walk alongside you from your very first call through your first day of services and beyond.
All services are funded through Arizona's Division of Developmental Disabilities (DDD). If you're not yet enrolled with DDD, we can help guide you through that process too.
Who We Serve
Eligibility Requirements
The Little Foundation serves individuals who meet the following criteria:
- Diagnosed with a developmental disability (intellectual disability, autism, cerebral palsy, epilepsy, or other)
- Enrolled with or eligible for Arizona Division of Developmental Disabilities (DDD)
- Residing in or planning to reside in the greater Phoenix/Surprise, AZ area
- In need of residential, employment, or community-based support services
